1)	Scan QR Code – it will bring you to the Parent Login
2)	Either log in with previous account, create an account, or continue as guest
a.	If you continue as guest, you cannot go back and edit information or look up account if there’s an issue
3)	Enter Athlete name and Student ID number
4)	Fill out “Medical Emergency Card” – Click both “I Agree” boxes after signature
5)	Fill out “Insurance & UIL Signature Page” – Click both “I Agree” boxes after signature
6)	Fill out Return to Activity Form – Click both “I Agree” boxes after signature
7)	Bring Physical AND Medical History form to Athletic Trainers
